Hydroxyzine for Cholinergic Urticaria Hives

Stronger’s Updates:
– The hydroxyzine worked for 2-3 weeks… back to running experiments.
Hi all, and welcome to my experiment thread. My real name is Jeremy and Ive had Cholinergic Urticaria since the fall of 2008. This thread is mostly for my benefit, so I have record of my experiments, but hopefully can be a useful learning tool for others.
~Stronger’s Cholinergic Urticaria Experiments~
Type of Treatment Refractory period (days) Date
1. Zyrtec + Laying out in Sun (30mins) 14 days April
2. Running+ Sun (30 mins) 3 days May
3. Running + Minimal sun (30 mins) 1 day June
4. Walking + intense sun/heat (60 mins) ~1hour June 15th
6. Laying in sun + Zyrtec 1/2day June21
7. Running everyday 3 days June24-June27

9. Running (Round Two) To Date: 3days July 14th
~Stronger’s Detailed Descriptions!~
7. Running Everyday
Intro: I read from several sources that running/phsyical activity in the sun might be the answer

consistent sunlight exposure, i would try to sit out in the sun almost every day i could. -hivesblow

Well after 4 to 5 days of being in the sun in the last 2 weeks, im cured of my Cholinergic Urticaria. 3 of days were spent tanning and swimming and the other were spent playing basketball. -jamie2346
I’ve tried numerous attempts at getting more sun exposure. My first major attempt resulted in a 1.5 week remission, but after that… one day exposures ceases to yield such long term results. I remember, before I had Cholinergic Urticaria, I use to run practically everday, then I took a break over the summer. The following fall Cholinergic Urticaria came. The idea behind this experiment is to look for a difference between one shot exposures and long term exposures. So without further rambling, here’s my running diary…
Day 1: I ran 30 mins, breaking out in hives needles during the run 3-5 mins in, but needle sensation stopped at ~15 mins. Took a shower immediately afterward and used body wash. I feel refreshed, minus a mild headache (may still be residual from Zyrtec overdose?). No outbreaks so far, but I feel occasional “flares” of hive pain. These flares last for >1 second. They are much more tolerable than typical breakouts. About to go on a sunny walk for “testing”.
Day2: I ran
Day3: I ran
Day4: I took a break on Saturday
Day5: Sunday took a break hives started to come back
Day6: INSANE HIVES attack Ouch
Day7: Saw Doctor
8. Hydroxyzine
After just a two day hiatus of running my hives came back x3fold. At this point a decided to give medicine another shot. I was prescribed hydroxyzine. The first 2 days I saw minimal results and moderate sedation effects… on the third day I experienced ample reduction of hives/itching. After 2-3 weeks the hydroxyzine effects seemed to wear off. I started becoming more and more itchy. One night I couldn’t get to sleep because of powerful itching. So now I’m back to running.
9. Running (Round Two)
Since hydroxyzine is starting to wear off, I’ll continue to use it, but I’ll also start running at a minimum of once every other day. I learned from my previous experience that any more and the hives will come back and painfully. I’m on my third day right now. Hives reduced to 90% again. I’ve noticed odd misquote sized bumps on the undersides of my arms. They are not red, but instead clear, and seem to be filled with water. I can pop them given adequate pressure which releases the “watery” substance (I’m not sure if it’s just water, but it appears to be just water). This increases my belief that Cholinergic Urticaria has something to do with problems sweating.

Congrats on your lifeguarding training! That sounds awesome. I always thought lifeguarding seemed like a fun job, and good luck with everything.
This should definitely show you whether or not Cholinergic Urticaria is directly connected with sun exposure or not. I imagine you will be getting your share of heat & sun exposure now for sure.
As far as your table, that is excellent. I think it is a great idea for everyone to jot down a simple table like yours to keep track of things like sun exposure.
I definitely plan to do the same once I can begin my sun experiment trials. I have been trying to get some more sun lately, but it has been really isolated times. To make it worse, it has been really stormy all week. I think it is going to clear up over the next week or two, so hopefully I can get some more sun.
But I will definitely get outdoor exposure DAILY once I get a house within the next few months. Then I will be able to tell for sure, and possibly even cure Cholinergic Urticaria once and for all.
Thanks again for the awesome update, and congrats again!

Title: Hydroxyzine for Cholinergic Urticaria Hives
Post by: StrongerThanHives on July 05, 2009, 10:23:25 PM
——————————————————————————–
I’ve been taking Hydroxyzine for a week now, since last Monday, after visiting my doctor.
I have been weightlifting almost every day.
Haven’t had the time to go running, or get much sun, except what’s necessary to complete daily tasks.
Over the past week…. my symptoms have dropped by (ballparkestimate) 90%. The first day I didn’t experience too much difference, but at 2-3 days my symptoms started to reduce.
Zero major outbreaks.
No hives unless I’m doing an activity I am commonly use to getting Cholinergic Urticaria with (like physical work in a dusty environment or prolonged exposure to hot sun, but even then it is only mild stinging.
At first the Hydroxyzine made me VERY sleepy and out of it… but now I the side effects are much more tolerable (and hopefully will diminish in time).
Hydroxyzine is also used to treat GAD (Generalized Anxiety Disorder) which, now that I think about it, matches a lot of symptoms I’ve been experiencing lately.
I like that I have found a drug answer to Cholinergic Urticaria. I remember when the doctor first told me to take Zyrtec and it didn’t work. I wanted to cry. I started running again and got into sun exposure, but I still worried about what winter would bring without sunny days to soak up the sun.
I had a pretty active fourth of July… and I had taken H earlier that usual so I wouldn’t be drowsy at all at the party. The next day I was busy so I didn’t get a chance to take my pills. I started itching for no reason (aside from the fact that our downstairs air conditioning was broken) and realized I had forgotten to take my pills.
This isn’t conclusive evidence, but it leads me to further believe that I have found a sucessful drug cure.
Now the only thing to worry about it the potential of building up a tolerance, or likewise having the effectiveness of the drug diminish.
Wish me luck x]
Anyway in Summary
Treatment Type: Drug, Antihistamine- Hydroxyzine
Treatment Time: 1 week
Reduction in Hives: 90%
Other Factors: Not really

Thanks for the update!
I hope that it continues to work well for you.
I have tried hydroxyzine and it did work well for me for a while. Eventually, it seemed that once I adapted over the course of about 2 weeks, it lost its effectiveness. But it may not be the case with you at all, and it may help you manage your cholinergic urticaria quite well.
It is good that it seems to already be working well. So I hope you can continue to manage it and keep the hives away with the hydrox.
Thanks again for the update. Just remember, you can beat this thing. We all can. As your name suggests, we are “stronger than the hives.”

Thanks for the replies.
Things have only been getting better. Less drowsiness + less hives. I didn’t feel any hives today… except 1 or two stings, and that was after taking a shower.
Tomorrow I’m going to be doing a lot of physical work that might normally incite hives attacks. This will be a good test to see how efficient hydrox is at hive prevention so far in the treatment.

Did a LOT of heavy lifting. Took some hydrox an hour before. Experienced some itching/stinging in the beginning, but when I really started to heat up, about an 1 hour so in, the hives stopped.
Did not experience any itching sensation until I got home and took off my shirt.
Going to take a shower now… hopefully I won’t be very itchy afterwards

The effects of hydroxyzine seem to be wearing off. I took more pills than usual and I’m still VERY itchy. I couldn’t get to sleep last night because I was so itchy. I’m still itchy. I have things I need to go do but I’m really tired and I want to go take a fufilling nap, but then the bed starts to heat up from my own body heat and then I start to heat up and then just from that, no covers or anything I’m gettting itchy.
It’s driving me crazy.

Sorry to hear that Strongerthanhives.
That almost echos my experience with hydroxyzine exactly! It worked well for about a week or so (during which time I was extremely drowsy daily), and as soon as the drowsiness wore off, the hives returned as usual.
That is why I eventually stopped taking them.
I am sorry to hear, but stay positive. Your struggle in this is far from over, and I think we are getting closer and closer to finding the best ways to not only manage, but eventually eliminate Cholinergic Urticaria.

Hi, have you tried out “Fexofenadine (Hydrochloride)”?
I’ve been taking this under 180mg regularly now over the last 2-3 months and haven’t had a severe attack since a week after use. I don’t doubt that the warmer temperates have helped greatly, but along with exercising on a regular (at least every other day – enough to make me sweat) as well as going outside often things have been much better than in the past. In the UK this can only be prescribed from your doctor, but repeat prescriptions can be re-ordered from your GP.
Don’t get me wrong, I’m not saying that my hives will never return (especially in the winter, as has been the case these last 5 or so years now), but it surely cannot be a coincidence that they’ve stopped for now shortly after taking them. I now take a pill every other day.

Title: Update
Post by: Joeshep on July 26, 2009, 06:07:49 AM
——————————————————————————–
Hello all. Just thought I’d update you all on my ‘progress’. I’ve been to the doctor about 4 times over the last couple of months. Initially the doctor prescribed Fexofenadine Hydrochloride which seemed to help a bit but gave me very noticible side effects such as diorhea and extreme drowsiness. The next time I went I saw a different doctor who seemed to be a lot more open minded to what I was describing which grabbed my attention some what. The doctor gave me a lot of information on the condition. Is there actually a procedure to being ‘diagonsed’ with something cos as of yet no doctor has said ‘you have Cholinergic Urticaria’? Or I don’t have any written conformation.
The doctors just seem to acknowledge it could be that :-. Along with all the printed out information the doctor gave me (which to be honest i’ve read tons of times before in my own time), she prescribed me to Cetirizine Dihydrochloride, which to be honest had no effect what so ever. So I leave it a couple more weeks and make another appointment. This time when I go I see the original doctor I saw for the second time. I went in with a ‘not leaving until you give me something friggin’ effective’ attitude and it seemed to have worked. He listened a lot more and generally seemed to be a lot more enthusiastic to what I was saying, which in turned made me a lot more enthusastic. Then comes the infuriating part.
He basically said ‘I think it could be Cholinergic Urticaria’, and ran me through what it was! He asked me what I thought it was the first time I saw him but he just blew it off. So I explain that I don’t think antihistimines are going to work and asked if there was any other form of treatmeant or if I could see a specialist. He then turns round and says that a specialist would just tell me the same thing he told me :- So very very reluctantly I agree to try yet more antihistamines. This time, Hydroxyzine Hydrochloride. He basically said that eventually one will work. Honestly the way he said it makes me feel like I’m doing a lucky dip! To be fair this recent antihistimine does seem to be more effective than the others. I’m still having outbreaks but they don’t seem to be as intense. But we shall see. I don’t know what I’m going to do if these arn’t effective.

Hello Joeshep,
Thanks for the update. Man, I can totally relate with your frustrations 100%. I recall feeling so frustrated, I just wanted to go and demand they do something. I can’t help you is NOT much help coming from a doctor when you are suffering through the annoyances of cholinergic urticaria.
Also, I can relate with your frustrations about the antihistamines. Every Cholinergic Urticaria person is different in that regard. Some can totally manage their Cholinergic Urticaria with a simple zyrtec. Others have to use a combo. And for some of us, we don’t get much relief at all from antihistamines. I do think you should continue trying to work with different combos with your doc to see if anything works.
And it is sad, but also rather true, that probably most of us on here have more of an “education” about cholinergic urticaria than many doctors out there. This is super frustrating, because when we go to the doctor they either don’t have a clue about Cholinergic Urticaria, or they only know what we know.
Anyway, thanks again for the update. I really hope and pray you can find something that helps! Good luck, and let us know if anything changes.

Hmmm well this latest medication seems to be wearing off now. Initially it started working really well. Didn’t completely stop the attacks but blocked them to a noticible extent. I don’t know whether to just go it alone now. All this medication is just giving me false hope.
Also, has any one researched homeopathic remedies? I read a very interesting blog on it ealier. Am I able to link it to these forums?

I am sorry about it wearing off. You may want to keep working with your doctor to see if it helps, but I too don’t get a lot of help from antihistamines.
About homeopath stuff, here is a quick summary of all I have read about/tried over time:
1. Acupuncture: some find it helps a little, some think it is worthless. I have never seen that it has cured Cholinergic Urticaria (and if anyone did get relief I would think it was merely coincidence). It probably wouldn’t be worth the cost.
2. Cleanses (colon/liver, etc.)– I have tried anti-candida stuff, I have fasted, and much more. Nothing helped here, and no one else has been cured trying these things.
3. Zappers–I just wrote a blog post on this, I think it is absurd.
4. Special herbs/remedies–If it really worked, I think we would all know. Many people have tried anti-worm/parasite remedies, herbs/vitamin/supplements that are supposed to help with hives, etc. So far, it all seems mostly fruitless in my opinion.
5. Creams–I don’t think any homeopath cream would even change Cholinergic Urticaria a bit. Perhaps some real medicated steroid type cream would change something (although there would be risks involved), but nothing homeopath would help that I have ever heard/tried.
I think that covers most all homeopath things. Again, you are always welcome to try something (under a doctors approval). But I just think you would end up getting disappointed and wasting your money. I would stick with a doctor. Also, I know how it is wanting so badly for the hives to stop and getting frustrated when nothing works. But we must stay positive and keep fighting. It is natural to want to fall victim for the homeopath “miracle cures.” But it probably just won’t be worth it.

Dear StrongThanHives,
Thanks a lot for the table, makes sense and the results are even more meaningful. I have been trying to get rid of this itch which is with me 24/7 and gets worse in the sun. But I have had zero positive effects even after walking gently in the intense sun in the hot climate of India, where I reside presently. How do you cope with this itch? Thanks,

I was going to make a post but I then realised that all my posts on this forum are in a way my a log.
As of now, I continue to exercise on a daily basis. I’m still taking the histamines so the next step would be to see if this does do anything positive by not using it and performing exercising ONLY for a day (or longer if things go well). Exercise is definitely helping. I’ve varied my routine by increasing the difficulty of my exercise (e.g. shorter rest times, performing more reps) which shortens the time required to sweat and have even plucked the courage to do so with the radiator on (which again does make it easier for me to sweat).
@ jamesmacin
Lets use the scenario that your lack sweating is the cause of your hives. As you should have gathered by now, the best way of sweating IMO is through exercise (cardio exercise to be more specific) and being in a hot climate. Before I continue, I have heard many people say that their hives only occur in the summer (hot climate) and not in the winter (cold climate) which would be the reverse for me but lets just pretend/assume for now that we have the same issues.
During the winter, just sitting in a hot room would cause a strong hives reaction for me. This was the same for me initially when the temperatures raised soon after winter had passed. I could only assume that this was because my sweat pores were “blocked”, to the extent that the sun alone wasn’t enough to make myself sweat. Exercise (AND heat) IMO is the best way of “unblocking” your sweat pores. Once this is one you will sweat profusely even by the sun alone.
Examples of this theory being true come from my ability to now exercise/sit/sleep in the same room, with the same radiator on at full blast throughout the day without problem. Last year this was impossible. Many others on the forum have said similar things (in that exercise has helped them). I’ve also heard of people using sauna’s which have had a temporary improvement on their hives.
i hope this helps.

So I went to my allergist yesterday got tested and all and like I suspected she thinks I may have Cholinergic Urticaria so she prescribed me some lotions and Hydroxyzine a antihistamine. I was wondering if anyone else has used this drug and can tell me if it helps? The doctor said to take either one-three daily, well being me I went for the three well today I went for a swim in my backyard and when I got out I was winded way more than normal, then I got all dizzy and weak and had to sit down, rest, and drink some water. So I guess I’m gonna cut down to one a day, I just hope it works because I have still been itching.

Yep, it was the first antihistamine I was prescribed.
It knocked me out cold for the first few days, and I slept like a baby. It made me extremely drowsy and groggy.
I thought it was working great, but then, as soon as I stopped being drowsy (about a week later), the hives came right back. I continued to take it for a week or two after, but I eventually gave up and stopped.

Be extremely careful with hydroxyzine. My doctor had me on too high of a dose and it turns out that it is also an anti-anxiety drug. I stopped giving a care about everything in my life and really took a toll on my grades. I’m on 25mg a day of it right now and I’m alright though but I’m not sure how much good it actually does. Just keep in mind that it does other things besides being an anti-histamine.

Title: Your gonna want to read this! **HOW TO SUPPRESS Cholinergic Urticaria 100%**
Post by: Darkhorse on March 20, 2011, 01:18:58 PM
——————————————————————————–
What you need;
– Sodium Ascorbate*
– Niacin** (Vitamin B-3)
– B5 (twice a day)
– Hydroxyzine***
* = The alkaline form absorbs better than it’s acidic version ascorbic acid so if taking ascorbic acid you may need a higher dose
** = Has to be the flushing niacin as B3 not nicotinic acid or niacinamide
*** = Hydroxyzine is a prescription first generation oral anti-histamine which needs to be prescribed, mention it to your GP/Dermatologist
How do these supplements work for Cholinergic Urticaria?
– Sodium Ascorbate is used to help dissipate blood histamine levels
– Niacin stimulates the release of histamine from the mast cells in doing so we can deplete the histamine stores
– B5 (pantothenic acid) can help interferer with the conversion of histidine into histamine
– Hydroxyzine works as a potent H1 receptor inverse agonist
I had Cholinergic Urticaria since April 2010 and it continued to get progressively worse until it severely effected my quality of life
It is now 100% suppressed by following this plan
I also have a diploma in nutrition which is where I have a vast array of knowledge in relation to the effects of supplementation on histamine levels
Drop me a post if you have any questions
I am very busy at the moment, I will post back here again next week and post up the references when I get a chance
– Paul
NOTE: Please do not take any of these medications without talking to a doctor first, and getting approval and the proper doses

Title: Hydroxizine, Ranitadine, and Omeprezole for Hives–I think i’m fixed
Post by: Fregien on February 08, 2012, 09:48:01 PM

I have been suffering from Cholengeric Uticaria for 17 years. It came upon suddenly. I was doing step aerobics when my newborn was about 9 mo. old and suddenly my stomach and chest became hot and broke out in hives. It was odd. I took a picture of my skin, thinking wow, that’s weird…. From that day forward, it was a regular occurrence, getting worse each year. I have been blood tested, skin tested, even stool tested(kind-of embarrassing), steroided and allergy shotted to no avail. I’ve been to the best at the Mayo (here in Scottsdale), to the holistic, which felt crazy….
I’ve been on every antihistamine combo anyone can think of. Every supplement you’ve even considered. to no avail. For anyone out there who doesn’t understand the situation- LUCKY YOU! It is debilitating, uncomfortable, and most of all embarrassing, as WE all know. Going forward- Good news for me! Two weeks ago, my boyfriend wanted to go snowboarding with a friend and client of mine. I haven’t been snowboarding in over 15 years. My first thought-My hives. I can’t possibly go. Snowboarding is incredibly physical. My saving grace- At least i’ll be wrapped up in clothes and no one can see my freaky skin ( that is except my red hivey face).
I think I can get away from the group and hide at some point until the hives cool down. ( For me 30-120 min) I did an internet search a few days before the trip, just in case there might be some last minute cure I could try while on this trip. I can’t remember the post so I can’t take credit, but I found my drug combo that became my cure….so far. (THANK-YOU, whoever you were) It’s been 2 weeks, no hives, but, it hasn’t been more than 2 days before without hives, so I had to post. My new regimen- Omeprazole (Generic proton pump inhibitor- over the counter.)
I use this generic- one time at night. AM-1 hour prior to any exertion activity- 1 tab- 25mg Hydroxizine(prescription- will make you tired- Start on a weekend, you will get used to it) + 1 Tagement(Ranitidine) 150 mg tab. That’s it. I haven’t had hives since. This week- Saturday- 4 mile hike- up and down mountain hike- very hard(for me)- I was breathing hard- couldn’t catch my breath, hot….. no hives. My skin was red. I could see my skin react underneath. red, but no itch, no hives and my face looked normal. Sunday- Hike- 3 miles- Camelback Mountain- strenuous- major loss of breath, red skin, no hives. Rest on Monday- My god, my body is not used to ant exercise! Tuesday- Pinnacle Peak Mountain again- 4 miles. Red skin. No hives. Not sure if this will help anyone else, but I am feeling great. Playing racquetball this weekend. Can’t wait! Hives F’ing Suck!! I hope everyone finds their cure…. And I hope mine lasts. Hope this helps anyone

Update- last post Feb. 8. Visited my regular doc last week to refill meds.
He doesn’t understand why the new drug protocol is working, but said,
Let’s not rock the boat and keep at it. To me it seems, no one knows what the hell
Is causing the problem, so any solution is viable. This is working
For me still. I’ve been hiking 3.5-5 miles at least every other day. Uphill strenuous
Red skin, yes, but lasts 10 min. I think normal people deal with that as well
Hives, no. It’s seriously life changing for me, as it would be to all of you with hives. I can’t help but recommend what I used about working for me.
